Bizarre though it seems, this is the most credible explanation I can see.
A gun accident (assassination attempt?) on him. Sure seemed to be pretty careless in that respect.
Almost as if all his people loved him and he didn't need to take any precautions whatsoever.

It would explain the secrecy.
He is insisting there are zero cases in NK.
It would be a tad embarrassing if the most prominent figure in NK contracted it.
