I don't know if this will help or not. I have a Polish surname index cd for the year 2003.
There are 2009 people with this surname distributed throughout Poland.
Because of the wide distribution, my guess is that it is true that this is a quite ancient name. I would only be speculating as to the origins but I can give you perhaps a few hints based on modern surname distribution of the Siwy surname.
The highest concentration based on population distribution is:
KaTG: M. 227, F. 194, KamPi: M. 87, F. 102, KamBy: M. 58, F. 58
Those are the top three, here is where they are: KaTG=Tarnowskie Gory, KamBy=Bytom, KamPi=Piekary Slaskie
These are clearly in the same area- Upper Silesia, and almost half of the people with this surname live within 10? miles of each other. The rest of the distribution is quite lower in numbers.
