In polite society in the USA, almost always frowned on and rarely used  by ANYONE.

It's used frequently by younger people, and not infrequently by many others.

Used to be almost always, today I would say usually. However, having said that there is sort of a double standard in tht more likely looked down on when coming from a woman, then from a man.

less so today then 40 years ago, and much more commonly used. 

Here in this part of Spain, considered lowclass, but does not even rate a flinch,

even when used by preschooleers, because in both places its normal context

is not necessarily sexual. The reason the offence in the U.S. is because America is

overly focused on sexual issues, most of which are non issues in the rest of the world.

I agree. Frankly, I think it has lost much of it's power--used so frequently that it doesn't mean much more than golly gosh a lot of times.
