order form?

I am really new to Notion and to this sub. I tried to search but I couldn't find anything.
Is there a way to make a simple order form in Notion? I just need people to enter name, contact and pick a few of given choices. Ideally it collects the info in either a new Notion page (list style) or google sheet or similar. No payment handling etc. I have weekly changing order forms and Notion is so easy to update I would love to have it all in Notion. Any idea if this is possible/how to do this?

With aidaform, you have the option of creating different tabs and pages if you would like to organize your form that way, or you can have all form fields visible at once, much like a paper form (Typeform alternative). Because sometimes you just want a clean-cut, fully visible form.
